ABSTRACT

In the elderly patients who complain of pain in the buttock and leg, it is not easy to distinguish whether the pain comes from the lesion of the hip or from the spine. A 78-year-old female who was treated conservatively for persistent pain in the right buttock and leg after an operation for spinal stenosis in the local clinic visited our clinic. Septic hip arthritis with severe femoral head destruction and multiple abscesses in the buttock and iliopsoas muscle were diagnosed 2 months postoperatively, and spinal abscess in the site of the previous operation was detected by a subsequent MRI study. To avoid such a delay of the diagnosis and treatment, it is important to suspect hip joint lesion earlier for the source of persistent pain after a spine operation. Further more, diagnostic evaluation is necessary to rule out co-infection of the spine or iliopsoas muscle when a hip joint infection exists.

ABSTRACT

BACKGROUND: We evaluated the clinical and radiological results of one-stage correction for cerebral palsy patients. METHODS: We reviewed clinical outcomes and radiologic indices of 32 dysplastic hips in 23 children with cerebral palsy (13 males, 10 females; mean age, 8.6 years). Ten hips had dislocation, while 22 had subluxation. Preoperative Gross Motor Function Classification System (GMFCS) scores of the patients were as follows; level V (13 patients), level IV (9), and level III (1). Acetabular deficiency was anterior in 5 hips, superolateral in 7, posterior in 11 and mixed in 9, according to 3 dimensional computed tomography. The combined surgery included open reduction of the femoral head, release of contracted muscles, femoral shortening varus derotation osteotomy and the modified Dega osteotomy. Hip range of motion, GMFCS level, acetabular index, center-edge angle and migration percentage were measured before and after surgery. The mean follow-up period was 28.1 months. RESULTS: Hip abduction (median, 40degrees), sitting comfort and GMFCS level were improved after surgery, and pain was decreased. There were two cases of femoral head avascular necrosis, but no infection, nonunion, resubluxation or redislocation. All radiologic indices showed improvement after surgery. CONCLUSIONS: A single event multilevel surgery including soft tissue, pelvic and femoral side correction is effective in treating spastic dislocation of the hip in cerebral palsy.

ABSTRACT

PURPOSE: This study was designed to verify intermediate-term functional and radiological results of limb salvage operation using endoprosthetic replacement system (MUTARS(R)) used in patients with a malignant bone tumor. MATERIALS AND METHODS: Thirty one cases which used MUTARS(R) tumor prosthesis were reviewed. The mean age of the patients was 49.2 years and the mean follow up was 39.8 months. We retrospectively reviewed complications, and evaluated functionally and radiologically by Enneking functional score, ISOLS radiological implants evaluation system at last follow-up. RESULTS: 3 patients had died of disease, distant metastasis was seen in 4 patients and local recurrence was seen in 1 patients. Complications were developed in 12 patients. (infection 6, leg length discrepancy 2, aseptic loosening 2, periprosthetic femoral fracture 1, screw loosening 1) Mean value of total functional scores were 81.2% in proximal femur, 77.4% in distal femur, 78.1% in proximal tibia, and 80.2% in proximal humerus. The overall radiological result was relatively satisfactory. CONCLUSION: Our results suggest limb salvage with the MUTARS(R) endoprosthesis is successful with good functional and radiological results. But we should be careful with complications such as infection.

ABSTRACT

Multiple rice bodies are commonly observed in various rheumatologic disorders and tuberculous arthritis, but rice bodies are rarely observed in nonspecific synovitis without any underlying disease. We performed arthroscopic rice body removal and synovectomy for nonspecific synovitis with multiple rice bodies in a 31-year-old male patient, and the results were satisfactory. No evidence of underlying disease, including rheumatoid arthritis or tuberculous arthritis, was observed during 2 years on the follow-up evaluations after the arthroscopic procedure. There have been very few confirmed rice bodies cases without underlying disease, so making a careful differential diagnosis to rule out rheumatologic and infectious disorders is required to avoid possible aggravation of disease, which may result in persistent articular cartilage destruction.

ABSTRACT

PURPOSE: To evaluate the radiologic, clinical results between who had intertrochanteric fracture, treated with Compression Hip Screw (CHS) and Proximal Femoral Nail Antirotation (PFNA). MATERIALS AND METHODS: We retrospectively reviewed each 36 and 48 patients of intertrochanteric fracture which were treated with CHS or PFNA by one surgeon from January 2005 to June 2009. We evaluated mean operation time, amount of bleeding, radiologic results, and the clinical outcomes with the mobility score of Parker and Palmer, social function scoring system. RESULTS: The mean operation time, amount of bleeding were less in the PFNA group, there were 116.7 min, 486.1 ml for the CHS group versus 87.7 min, 289.6 ml for the PFNA group. The radiologic results were not significantly different. Decrease of mobility score of Parker and Palmer, social function score were similar. Proximal migration of leg screw and perforation of femoral head was 2 case and deep infection was 1 cases in CHS group. CONCLUSION: There were no significant differences that are clinical and radiological results in treatment of intertrochanteric fracture using the CHS and PFNA. But PFNA is less invasive device than CHS, therefore it may be useful device in elderly patients.

ABSTRACT

PURPOSE: We compared the results of electromagnetic navigation assisted total knee arthroplasty (TKA) with that of conventional TKA from the viewpoint of the postoperative limb alignment and the implant position. MATERIALS AND METHODS: We retrospectively analyzed 115 consecutive total knee arthroplasties that were done between September 2006 and June 2007. There were 65 navigation assissted cases and 50 conventionally implanted cases. We analyzed the postoperative radiologic results, the clinical results over one year follow up after surgery and the range of motion (ROM). RESULTS: The postoperative mechanical axis deviation showed no significant difference between the two groups. However, the tibial zone, as assessed by Kennedy and White's method were at zone C for 60 cases (92%) in the navigation group and for 40 cases (80%) in the conventional group (p=0.01). On the coronal plane, the implant position of the femoral component was 89.4+/-4.1degrees in the navigation group and 87.6+/-4.3degrees in the conventional group (p<0.01), and the implant position of the tibial component was 89.7+/-1.1degrees and 91.9+/-1.8degrees in each group, respectively (p<0.01), and this shows the better result in the navigation group. CONCLUSION: Electromagnetic navigation assisted TKA had similar clinical results on the short-term follow up as compared with conventional TKA. However, it radiologically showed a more satisfactory position of the implant and it displayed superiority in locating the precise position of the femoral and tibial components, and especially on the coronal plane.

ABSTRACT

PURPOSE: The treatment results of a two-stage reimplantation of infected total knee arthroplasty with either the articulating or non-articulating types of antibiotic-loaded bone cement spacers were compared and analyzed. MATERIALS AND METHODS: Between July 1995 and October 2005, this study reviewed 29 patients who underwent two-stage reimplantation of infected total knee arthroplasty and were followed up for at least 2 years. Of the 29 cases, 12 used articulating and 17 used the non-articulating type. RESULTS: Fifteen cases (52%) showed bacterial growth in the preoperative and intraoperative bacterial culture, and 14 cases (48%) had no growth. 27 cases showed no evidence of reinfection until the last follow up after two-stage reimplantation. One out of the 12 cases in the articulating group and 1 out of the 17 cases in the non-articulating group had a reinfection. The range of motion (ROM) of the knee joint and HSS knee rating scale increased more significantly in the articulating group than in the non-articulating group, and the degree of increase in the ROM of the knee joint and HSS knee rating scale was significantly larger in the articulating group. CONCLUSION: The two-stage reimplantation with an antibiotic-loaded bone cement spacer showed a 93.1% of success rate. The articulating group showed better results in the knee ROM and HSS knee rating scale than the non-articulating group

ABSTRACT

Galeazzi fractures in child is rare and seldom necessary of operative treatment because the result of conservative treatment is good. We present the patient who was a 11-year-old male and fell onto his both hands during a hundred-meter dash. His diagnosis was bilateral Galeazzi fractures and limited open reduction and internal fixation with Kirschner pins was initial treatment at local hospital. After 4 weeks postoperatively, Kirschner pins were removed and rehabilitating exercise was started. After 4 months postoperatively, he was transferred to our hospital due to malunion with severe angular deformities and distal radioulnar joint (DRUJ) dislocation. He was treated with corrective osteotomy. Thus, as in this case, we suggest more careful treatment and observation if conservative method of Galeazzi fracture in child is chosen and consider operative method as treatment according to age and pattern of fracture.

ABSTRACT

PURPOSE: To observe the changes in periosteum-derived mesenchymal stem cells and different scaffold materials transplanted into rabbit long-bone defects. MATERIALS AND METHODS: Thirty-five white rabbits were grouped according to the material transplanted into their tibial bone defects: Group 1 (control group); Group 2-A (agar plus mesenchymal stem cells); Group 2-B (agar only); Group 3-A (Biocompatible Osteoconductive Polymer plus mesenchymal stem cells); Group 3-B (BOP(R) only); Group 4-A (xenograft plus mesenchymal stem cells); Group 4-B (xenograft only). After surgery, radiologic and microscopic observation were performed weekly for 8 weeks. RESULTS: Rabbits transplanted with mesenchymal stem cells showed better bone formation than those without. Mesenchymal stem cells transplanted with agar had better results than mesenchymal stem cells plus BOP(R). Rabbits receiving xenografts showed severe inflammatory reactions. CONCLUSION: Further research is needed on rigid scaffold materials which minimize immune reaction, and on how to ensure uniform distribution of mesenchymal stem cells within such materials.

ABSTRACT

PURPOSE: This study investigated the results of cementless total hip arthroplasty using a circumferentially porous coated tapered femoral stem and a porous coated acetabular cup after a follow-up of more than 2 years. MATERIALS AND METHODS: 70 hips in 65 patients were followed up for more than 2 years after surgery who underwent total hip arthroplasty with a Versys Fiber Metal Taper femoral stem (Zimmer, Warsaw, IN) and Trilogy acetabular cup (Zimmer, Warsaw, IN). RESULTS: The mean Harris hip score increased to 96.2 points. There was no osteolysis in the acetabulum. There was no distal osteolysis in the femur but there was osteolysis observed in Gruen zone I in 1 hip (1.4%). Endosteal bone formation was observed in 67 hips (95.7%) mostly in Gruen zones II and VI. There was no subsidence of the stem. The survival with an acetabular and femoral revision for aseptic loosening was 100% respectively. CONCLUSION: Cementless total hip arthroplasty using a circumferentially porous coated tapered femoral stem and a porous coated acetabular cup showed excellent results after a follow-up of more than 2 years, inducing good bone ingrowth, as well as no migration of the cup and no osteolysis around the distal femur and acetabulum. However, a long term follow up is necessary.

ABSTRACT

PURPOSE: To study the effectiveness of chondrocyte and periosteum-derived cell transplantation in preventing bar formation in growth plate injuries. MATERIALS AND METHODS: Thirty immature rabbits were grouped according to site of growth plate damaged (distal femur or proximal tibia on the medial side) and type of cell and material inserted (chondrocytes plus agar, periosteum-derived cells plus agar, periosteum-derived cells plus Gelfoam, agar alone, or Gelfoam alone). Radiograms were taken for 16 weeks to measure the changes in varus angle of the distal femur and proximal tibia. Transformation of inserted cells was observed histologically. RESULTS: Rabbits inserted with agar alone showed more severe genu varum compared to those inserted with chondrocytes plus agar. Rabbits inserted with periosteum-derived cells plus agar or Gelfoam showed similar deformities compared to those inserted with agar or Gelfoam alone. Some inserted periosteum-derived cells began transformation into chondrocytes. All rabbits showed gradual bar formation and ultimate fusion of the growth plate. CONCLUSION: Cell-based therapy of growth plate injury still has obstacles to overcome. Further study will be required on how to maintain the inserted chondrocytes, or chondrocytes differentiated from the inserted periosteum-derived cells, and to prevent them from changing into osteoid formation cells.

ABSTRACT

PURPOSE: To compare the results of the thoracoscopic approach and open thoracotomy retrospectively for scoliosis requiring anterior release and fusion followed by posterior correction. MATERIALS AND METHODS: Of 22 rigid scoliosis patients, 10 open thoracotomy cases (group I) were compared with 12 thoracoscopic cases (group II) in terms of blood loss, operation time, and chest pain (visual analogue scale, VAS), as checked 1 week after operation and final follow-up, correction ratio of major curve and complications. Posterior correction was performed 2 weeks after anterior release. Statistical analysis of group differences was performed using the t-test or the Mann-Whitney. RESULTS: Blood loss was higher in group I, 268.0 mL (200-530) than in group II, 195.0 mL (100-280) (p=0.047). The operation time was shorter in group I, 108.0 minutes (90-180) than in group II, 175.0 minutes (120-240) (p=0.001), and chest pain (visual analogue scale, VAS) which was checked at 1 week after operation and at final follow-up was milder in group II, 3.9 (3-6), 1.6 (0-3) than in group I, 5.8 (4-8), 3.1 (1-5) (p=0.005, p=0.013, respectively). The correction ratios of the major curve between two groups were similar. The cosmetic aspects of the thoracoscopic approach were favorable. The postoperative complications were 1 pneumothorax in each group and 3 chronic chest pain in group I. CONCLUSIONS: The results of thoracoscopic approach for anterior release were compared favorably with standard open thoracotomy in terms of blood loss, chest pain both postoperatively and at final follow-up, and cosmetics, but unfavorably for operation time.
